Paragone Solutions is seeking a Case Manager to provide individual medical and dental readiness support to the Minnesota Army National Guard. The non-clinical CM provides technical expertise to medical personnel toward identifying, evaluating, resourcing, monitoring, and determining dispositions for service members who require assistance in the maintenance of health and the prevention or management of illness, injury, or disability. Case management knowledge domains consist of assessment, planning, implementation, coordination, monitoring and evaluations of the options and services required to meet the ARNG Soldier’s health and human services' needs. The core hours are onsite Monday-Friday, 0600-1700 at the Cottage Grove Armory.*This position is contingent upon contract award.Responsibilities
Administration, Coordination and Record Management:
  • Maintain soldier medical and dental records in accordance with Army Regulation (AR) 40-66.
  • Mark and safeguard files, output products, and storage media per classification level.
  • Create, maintain, and process medical and dental records for incoming, current, and exiting members.
  • Review medical records for completeness prior to various events (e.g., Basic Military Training, Schools, Transfer, Discharge, Retirement).
  • Prescreen soldier medical records for scheduled Medical Readiness Events (MREs) to ensure deployment eligibility.
  • Conduct quarterly medical/dental record inventory of soldier files.
  • Scan and index all medical documents received during tasks.
  • Run and provide new systems generated reports as new DOD Systems software becomes available.
  • Assist service members in scheduling appointments with appropriate medical and dental facilities.
  • Attend monthly eCase reviews to discuss open medical cases.
  • Coordinate Soldier treatment plans with appropriate medical facilities.
  • Schedule profile exams/evaluations and audiology exams.
  • Check daily the generated automatic referrals list from MEDCHART DOD systems after every MRE.
  • Correspond with soldiers to obtain information for the Soldier’s case management file.
  • Track medical appointments and ensure Soldier attendance.
Case Management:
  • Interview soldiers and document communication in the eCase module.
  • Perform charting, tracking, and documentation in the Medical Data for Care History and Readiness Tracking (MEDCHART) applications.
  • Manage Soldier’s care from identification of the condition to either return to duty or administrative separation.
  • Educate soldiers on resources to improve medical and dental readiness.
  • Review and track health assessment lab results and educate soldiers on lifestyle modifications.
  • Update MODS as required on various aspects, including immunizations and pregnancy designation.
  • Provide liaison services and coordinate with the DSS on challenging case files.
  • Ensure Soldiers with specific profiles are brought to the attention of the State Surgeon Office or representative.
  • Prepare medical documentation for Line of Duty (LOD) investigations.
  • Lead soldiers through the medical board process.
Documentation and Reporting:
  • Provide all medical and dental documents for case management to Medical Command.
  • Review electronic data in DOD systems before Medical Readiness Events (MRE) for early identification of non-deployable soldiers.
  • Review all open cases regularly to ensure tracking, management, and communication.
  • Post-MRE, run and provide reports to the DSS.
  • Monitor MEDPROS to ensure all scheduled services are completed and current medical status is reflected.
  • Mail all medical records to ensure timely and accurate processing of medical records to and from mobilization sites.
Compliance and Training:
  • Follow regulations and policies applicable to Individual Medical Readiness (IMR) and deployment.
  • Comply with and maintain all Health Information Portability and Accountability Act (HIPAA) standards.
  • Attend and perform required services at all MRE and SRP events.
  • Complete HIPAA certification course on a yearly basis.
Requirements
  • Must be a Registered Nurse (RN), Licensed Vocational/Practical Nurse (LVN/LPN), Social Worker, Army MOS of 68 or 42 series level 4 as comparable credentials or AOC Series 67-73 with degree, Care Coordinator with six years’ experience or equivalent experience, knowledge and/or abilities.
  • Must possess a degree from an accredited college or university. It’s preferred that the candidate have at least one year of case management experience or at least five years of health care experience.
  • Security - Must be able to pass a national security background check for access into military electronic systems.
  • Must maintain active and valid HIPAA certification.
  • Must be able to operate a personal computer to input, store, retrieve and manipulate data for various reports as required.
  • Must be able to use various software programs to maintain database files and prepare reports, including Microsoft Word, Excel, PowerPoint and Outlook.
