- Support agency services by managing incoming leads, verifying and documenting third-party payers, preparing patient intakes, and reporting daily to the agency Administrator on the status of all leads, inquiries, and admissions.
- Greet patients immediately and warmly, both in person and on the phone
- Answer all incoming calls promptly and politely; screen calls and take/relay messages, as necessary.
- Instruct clients on completing new client forms and advise clients of their fiscal responsibility.
- Perform the initial client intake and evaluation, develop the care plan, and perform customer service visits in the client’s residence.
- Provides documentation of client intakes, service agreements, consents, care plans, customer service visits, and any other necessary forms required to initiate care.
- Coordinates services between the client and the agency
- Informs agency personnel of changes in the condition and needs of the client.
- Teaches and counsels the patient and family/significant others regarding their service needs and other related problems of the patient at home.
- Attend staff meetings, patient care conferences, and in-services as scheduled.
- Understands and adheres to established policies and procedures.
- Provides services and non-medical treatments as assigned.
- Develop and implement sales and marketing strategies that capitalize on area market opportunities in coordination with the agency Administrator.
- Develop referral relationships with significant accounts assigned to, including large hospitals and health systems, major physician practices, and other potential significant referral sources.
- Meets or exceeds established productivity goals and goals for hourly services growth.
- Represents the company appropriately to groups, community organizations, and the public.
- All required duties asked by the supervisor in the effective execution of your role as a Private Duty Care Coordination Liaison
- Reports all grievances and complaints patients and families make to the appropriate persons.
- Reports all allegations of abuse and misappropriation of patient property.
- Reports on all accidents and incidents observed.
- Identifies and responds appropriately to emergency situations.
- Observe the safety needs of the patients.
- Maintains confidentiality of necessary information.
- Thinks and acts calmly and logically to meet unusual occurrences of the job.
- Manages time efficiently and effectively.
- Promotes positive team image and communication.
- Works in collaboration with local office management teams and marketing/sales staff.
- Provide role modeling in teamwork, accountability, and a positive attitude.
- A High School diploma or GED is required.
- Preferred to possess state CNA licensure at minimum.
- Strong oral and written communication skills in Excel, Word, and CRM management.
- Excellent presentation skills.
- 1-3 years in patient care, sales, or a related field
- Demonstrated proficiency in in-home care marketing/sales preferred.
- Regular local travel (weekly) in all reasonable weather conditions.
- Primarily medium, exerting up to thirty-five pounds of force to lift, carry, pull, or move objects.
- Involves sitting, walking, standing, driving, talking, hearing, and reaching.
- In and out of offices, referral sources, and client residences throughout the workday.
- Exposure to disagreeable conditions, including inclement weather, odors, infection, illness, and physical contact with disruptive patients.
- Must be flexible to accommodate the completion of tasks, operational needs, and referrals.
