OverviewCoordinate Care. Support Compassion. Empower Hospice Teams.We are seeking a Hospice LPN Patient Care Coordinator to manage the day-to-day operations of our hospice programs and coordinate care services across clinical teams. This pivotal role supports timely and compassionate patient care by scheduling visits, facilitating interdisciplinary team meetings, verifying eligibility, and assisting with skilled nursing care as needed—all while ensuring compliance with regulatory standards.Key Responsibilities:Schedule visits requested by Registered Nurses, Social Workers, and Chaplains at the start of each benefit periodManage rescheduled, declined, missed, or reassigned visit requests and ensure visits are completed timelyPrepare and facilitate interdisciplinary team (IDT) meetings with complete documentationAccurately gather and enter referral information into hospice systems promptlyCoordinate durable medical equipment (DME) pickup and deliveryNotify appropriate staff at patient death or discharge, including pharmacy and physicianVerify Medicare eligibility and secure authorizations or reauthorizations as neededManage patient transitions by obtaining transfer documentation from previous agenciesProvide skilled nursing care within LPN scope under RN supervision in patient homes or inpatient units as neededRespond professionally and compassionately to referral inquiries, triaging clinical concerns within scope of practiceMaintain accuracy of patient benefit periods, events, insurance verifications, and physician/facility updates in systemsAdminister patient satisfaction surveys and maintain related documentationManage scheduling logistics including service codes, visit dates, clinician assignments, and PRN visit schedulingAbout YouQualifications – What you'll bring:
  • Degree from an accredited Practical Nursing program with current LPN license in the state of employment
  • CPR certification
  • 3–5 years administrative experience, with at least 2 years clinical experience in hospice or home health
  • Valid driver’s license, automobile insurance, and reliable transportation
  • Knowledge of hospice care concepts, terminal illness, and the roles of interdisciplinary team members
  • Proficiency with nursing assessment tools, Microsoft Office, and electronic medical records
  • Strong communication skills with compassion for patients and families facing end-of-life situations
  • Ability to work in an office environment with occasional travel to patient homes or institutional settings
