- Collaborate with interdisciplinary health care teams in diagnosis and treatment of Kidney/Pancreas and Liver Program patients as a member of the Transplant Surgery team.
- Manage the transplant population including evaluations and preoperative care, as well as follow-up postoperative care and coordination of on-going care.
- Develop expertise in cross continuum care management and treatment interventions specific to this patient population.
- Educate patients and families on preoperative and postoperative care.
- Facilitate and communicate with patient, family, and other care organizations to promote continuity across the continuum of care.
- Evaluate quality outcomes for the patients under the care of transplant surgery care teams through participation in the development of quality indicators and outcomes.
- Collaborate with other consultative care teams and the primary team in the management of Kidney and Liver diseases.
- Perform comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive, and psychosocial status.
- Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicate and document information.
- Develop pharmaceutical and non-pharmaceutical treatment plans.
- Monitor effectiveness of plan of care
- Perform procedures as trained and appropriately credentialed.
- Document within the Electronic Medical Record (EMR) and complete other necessary documents.
- Collaborate and/or consult physicians and other health care providers as appropriate.
- Educate, counsel, and assist patients and families regarding current health problems, health maintenance activities and preventive behaviors.
- Attend required training and meet key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission.
- Acquire information on the most current development and trends in the professional field through educational programs, in-services, professional associates, and current literature.
