AS THE SOCIAL SERVICE COORDINATOR, you'll be responsible for assisting patients and families in planning for post hospital care; to perform social assessment for discharge planning; to develop and maintain files on community resources and to provide direct social work services to inpatient units and outpatient services as necessary.Other information:
QUALIFICATIONS:
-Have Bachelor’s degree in social work or other related field
-Have one year of Social Work experience
-Have current Pennsylvania driver’s license if assigned to Home Health Care
-Private vehicle and active automobile insurance
-Strong organizational skills; critical thinking and problem solving skills
-Must be dependable and self-directed
-Have ability to interact effectively with patients, families and all members of the care team
