Primary Responsibilities:
Respond to patient needs during evenings, overnights, and weekends via in-person visits and phone triage
Deliver hands-on hospice care, including medication administration and comfort-focused nursing interventions
Guide families with empathy through symptom changes and end-of-life concerns
Maintain accurate, timely documentation using our hospice-specific EHR
Coordinate with interdisciplinary team members to ensure seamless care delivery
Educate families on the dying process and what to expect
Provide reassurance, emotional presence, and expert communication during crises
What We’re Looking For:
Active MN Registered Nurse (RN) license
Minimum of 1 year of hospice experience strongly preferred
Comfortable with travel and responding to calls across a wide metro area
Excellent at independent decision-making, prioritization, and triage
Compassionate communicator with patients, families, and team members
Tech-savvy enough to document and communicate from the field
Willing to participate in on-call rotations, including holidays and weekends
