- Assess patient's functional capabilities and home environment, identifying potential improvements and safety adaptations.
- Develop customized therapy plans based on each patient's specific needs, focusing on improving daily living skills and accessibility within their home.
- Guide patients and their families in the use and care of adaptive equipment and necessary home modifications.
- Document patient progress and therapy outcomes, maintaining accurate and comprehensive patient records
- Coordinate with other healthcare team members to provide holistic care that encompasses all aspects of the patient’s health needs.
- Monitor patient progress and make adjustments to the therapy plan as needed to ensure optimal recovery and independence.
- Provide education and support to family members and caregivers to facilitate understanding and help in effective implementation of therapy strategies.
- Adhere to all professional, legal, and safety regulations governing home health care practices, actively maintaining the dignity and rights of all clients.
- Current Ohio Occupational Therapy license in good standing (issued by the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board).
- Graduation from an ACOTE-accredited occupational therapy program (Master’s or Doctorate in OT).
- Passing score on the NBCOT (National Board for Certification in Occupational Therapy) exam.
- Current CPR certification (Basic Life Support, American Heart Association preferred).
- Valid driver’s license, auto insurance, and reliable transportation, as this role involves traveling to patients’ homes.
- Preferred: 1+ year of clinical experience, ideally with older adults, rehabilitation, or prior home health experience.
- Strong ability to assess functional limitations and develop individualized treatment plans.
- Knowledge of Medicare/Medicaid and OASIS documentation requirements for home health care.
- Comfort with electronic medical record (EMR) systems.
- Excellent communication skills for effective collaboration with patients, families, and interdisciplinary team members.
- Ability to lift or assist patients when needed and perform physical tasks related to therapy.
- Background check and health screenings (TB, vaccinations, drug screen) as required by agency policy.
