Upstream Rehabilitation is looking for an experienced Physical Therapist to join our Telehealth team!We are expanding our telehealth team and seeking a licensed Physical Therapist with expertise in both orthopedic and pelvic floor therapy to proveide virtual therapy to patients in our Oregon and Washington markets. What You Will Do in This Role:
  • Deliver high-quality physical therapy services via Upstream's telehealth platform (Raintree Connect), ensuring accessible and effective care for patients.
  • Perform virtual evaluations and re-evaluations to create and adjust personalized treatment plans with clear, measurable goals.
  • Collaborate closely with patients, caregivers, and interdisciplinary healthcare teams to support communication, monitor compliance, and optimize outcomes.
  • Instruct and guide patients through therapeutic exercises and functional activities to enhance mobility, strength, and overall function.
  • Maintain accurate, timely, and compliant documentation within the electronic medical record system.
  • Provide live or virtual mentorship and educational sessions to support the development of orthopedic skills and clinical excellence among less experienced clinicians.
Qualifications:
  • Graduate of an accredited Physical Therapy program.
  • Current or pending state licensure (OR and WA) in Physical Therapy (multi-state or compact licensure preferred).
  • Minimum of 2 years of clinical experience as a Physical Therapist in an outpatient setting; prior telehealth experience is a plus.
  • Completion of Level 1 Pelvic Floor training is preferred but not required.
  • Strong commitment to clinical excellence, with the ability to effectively communicate and mentor within a virtual care environment.
  • Reliable high-speed internet connection and a dedicated, professional workspace suitable for virtual patient sessions.
