Location: Petaluma, CAPosition Summary:We are seeking a dedicated and knowledgeable Physical Therapist (PT) to join our team in Petaluma. The successful candidate will deliver exceptional therapeutic care to patients by developing personalized treatment plans aimed at improving their mobility, strength, and overall physical function. This role requires a compassionate approach, excellent communication skills, and a commitment to enhancing the quality of life for patients.Key Responsibilities:
  • Conduct comprehensive assessments to diagnose physical conditions and develop individualized treatment plans.
  • Implement therapeutic interventions including exercises, manual therapy, and modalities to address patient needs.
  • Educate patients and their families on treatment plans, techniques for home exercise, and injury prevention.
  • Collaborate with a multidisciplinary team to ensure comprehensive patient care.
  • Maintain accurate and up-to-date patient records in compliance with regulatory standards.
  • Adjust treatment plans as necessary based on patient progress and feedback.
Qualifications:
  • Current state licensure or eligibility for licensure as a Physical Therapist in California.
  • Need a year of experience in an acute setting
  • Require Home Health Experience
  • Strong knowledge of physical therapy principles, procedures, and standards.
  • Excellent interpersonal and communication skills.
  • Ability to work effectively in a team-oriented environment.
  • Experience in a clinical setting is preferred.
  • Clean Driver's license.
