- Provide exceptional physical therapy services to patients by assessing and understanding the care plan.
- Evaluate patient's functional status and home environment for hazards or barriers.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Maintain and process all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
- Submits all clinical documentation, time records and other information in the required time frame.
- Licensed Physical Therapist (PT) in the state of employment.

- 1 year of clinical and home care experience is preferred.
- Graduate degree or greater in physical therapy.
- Oasis experience strongly desired.
- Have great communication and problem-solving skills.
- Have a valid driver’s license, reliable transportation and willing to cover an assigned territory.
- Experience with EMRs and utilizing a tablet and/or computer proficiently.
- Ability to work independently with minimal supervision.
- Some rotating weekend days required.
- Willing to work one (1) winter holiday and one (1) summer holiday per year.
