Job Description:SUMMARYThe InterRAI Assessor SSA conducts the State approved InterRAI assessmentas required by the DODD and at least once every three (3) years for redetermination of waiver services and authorizations. The Assessor is responsible for conducting the assessment following all guidelines set forth by the Rule and in accordance with established timelines. Full comprehension of the assessment is necessary to accurately identify needs.ESSENTIAL DUTIES AND RESPONSIBILITIES75%
  • Completes State approved InterRAI assessment as required by the DODD and at least once every three (3) years for redetermination of waiver spans;
  • Assists in the identification of appropriate service needs and budgetary allotment;
  • Communicates to the respective SSA the results of the InterRAI assessment and collaborates with other team members as needed ;
  • Works collaboratively with other SSAs and outside agencies to obtain necessary documentation;
  • Receives monthly assignments that require follow up and action to be taken;
  • Reports monthly the outcomes of assessments conducted;
  • Attends monthly meetings to review outcomes of completed assessments;
  • Follows up on completed assessments to assure integration into service planning;
  • Maintains knowledge of resources available in the community;
  • Assembles corresponding letters and due process forms for mailings;
  • Completes daily Targeted Case Management (TCM) notes to document all services delivered, TCM notes are subject to subpoena, Billable TCM notes are billed to the individual's Medicaid;
  • Adheres to work assignment timelines, including TCM;
  • Provides excellent customer service in order to support the Board's vision and mission.

  • Follows all State and Federal Rules as well as Board Policies and Procedures;
  • Provides crisis assistance to ensure that needed services occur in a critical or unexpected situation (arranging for respite, addressing behavioral or medical emergencies, on-call responsibilities, etc.);
  • Completes accurate documentation in keeping with requirements of the County Board as well as any applicable Medicaid requirements;
  • Reports major unusual incidents to include alleged abuse/neglect in accordance with HB 403, Ohio Revised Code 5123, Administrative Rule 5123:2-17-02, and County Board Policies and Procedures.

  • Assists individuals with due process when necessary;
  • Adheres to a flexible work schedule designed to meet individual needs.
An integral part of the job function is ensuring that services, supports and oversight of such supports occurs with the focus on the individual receiving services. Person-centered planning is the foundation for all services, supports, monitoring, supervision and coordination for the individuals served. Must have a valid Ohio Driver's License, an acceptable annual BMV Check, evidence of a good driving record, and proof of insurance.EDUCATION AND EXPERIENCEBachelor's Degree in related field.
