- You hold a current Ohio license as a Physical Therapist, Occupational Therapist, Speech-Language Pathologist, or Therapy Assistant (PTA/COTA) with management experience.
- You bring a minimum of three years of clinical experience in long-term care or skilled nursing, with prior supervisory or management experience preferred.
- You are knowledgeable about PDPM, MDS, and Medicare/Medicaid reimbursement guidelines.
- You have strong leadership, organizational, and communication skills and thrive in a fast-paced environment.
- You are able and willing to provide hands-on therapy services when needed to support residents and the team.

- Provide leadership and direction to therapy staff including physical therapists, occupational therapists, speech-language pathologists, therapy assistants, and rehab aides.
- Develop and manage therapy department goals in alignment with facility objectives.
- Oversee scheduling of therapy staff and caseload assignments.
- Ensure timely evaluations, treatment, documentation, and discharge planning.
- Monitor and manage therapy services to meet productivity and quality standards.
- Provide direct therapy services (evaluations and/or treatments) as needed due to staffing needs, high caseloads, or urgent resident needs.
- Collaborate with physicians, nursing, and interdisciplinary teams to support comprehensive care planning.
- Ensure therapy interventions align with residents’ individualized care plans and long-term goals.
- Ensure adherence to federal, state, and local regulations including CMS, HIPAA, and professional practice standards.
- Participate in quality assurance initiatives and audits; address deficiencies and implement corrective actions.
- Support hiring, training, supervision, and performance evaluation of therapy staff.
- Foster a culture of continuous improvement, professional development, and teamwork.
- Track department budgets, expenses, and billing practices.
- Monitor therapy utilization and ensure accurate documentation for reimbursement (e.g., PDPM compliance).
- Adhere to infection control policies, professional therapeutic services, and Community First Solution’s standards, policies, procedures, and guidelines.
- Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
