We offer an excellent salary and benefits package to include employer paid health insurance premiums for all full-time employees, PTO and paid company holidays, 403b retirement, EAP, and year-round employee wellness and appreciation initiatives.POSITION SUMMARY:This is a professional position that will provide Rapid Rehousing Intensive Case Management (ICM), wraparound facilitation, and supportive interventions to families and highly vulnerable individuals who are homeless and placed in hotel/motel or emergency shelter. The Rapid Rehousing ICM will develop individualized housing plans, facilitate initial evaluations, and monitor the delivery of services to facilitate placement into permanent affordable housing. The Rapid Rehousing ICM provides ongoing intensive case management services to include mental health services, PCHC, connections to daycare, transportation to needed service appointments, budgeting and support to financial stability etc. while working to move the family into and sustaining permanent housing. The Rapid Rehousing ICM links individual/families with community resources, provides supportive counseling, and intervention practices. This position will work closely with other service providers, both internally and externally, and funders to provide complete comprehensive services in the community setting. The Rapid Rehousing ICM is required to attend and participate in regular IDT staffing to review the housing plan and to address all identified barriers to securing permanent, safe and affordable housing. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. This position is field based. Hours are flexible, evening and weekend work is required. The Rapid Rehousing ICM will be required to become a high-Fidelity Wraparound Facilitator within 6 months of accepting the position.Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.Directions for Living is a High -Fidelity Wraparound Certified Agency and has engrained the evidence-based practice of high-fidelity wraparound into agency wide service delivery.EDUCATION EXPERIENCE/REQUIREMENTS: * Bachelor's Degree in Social Work or related area of study from an accredited university preferred.
  • Five (5) years' experience working with the homeless population and/or vulnerable children and families and/or three (3) experience as a certified Child Welfare Case Manager (CWCM) required. Seven (7) plus years of experience may be substituted for educational requirement. Must acquire knowledge of community resources.
  • Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must be able to use Telehealth platform for service delivery and HMIS.
  • Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection
