- Graduate of an accredited nursing program
- Current Registered Nurse (RN) license in the State of Nebraska; ongoing CEUs required
- CPR certification required (or willingness to obtain)
- Minimum of two (2) years of home health experience required
- Experience in long-term care or acute care preferred
- Must meet all qualifications to practice as an RN in Nebraska

- Comfortable working independently in both clinical and administrative environments
- Assist with patient evaluations and eligibility for home health services
- Support the smooth transition and coordination of new admissions
- Participate in “Start of Care” visits as needed
- Participate in planning and coordination of patient care
- Monitor and document changes in patient conditions
- Communicate effectively with physicians, caregivers, and the care team
- Serve as a treating clinician in the field when necessary
- Support clinical team operations and assist with day-to-day scheduling coordination
- Perform clinical record audits, track follow-ups, and assist in quality improvement processes
- Help develop or update care protocols and documentation standards
- Provide hands-on care in the home setting when needed
- Support coverage for open visits or during periods of increased patient census
- Participate in regulatory readiness, internal audits, and infection control measures
- Ensure clinical practices align with agency policies and industry standards
- Attend required team meetings and case conferences
- Keep the Director of Home Health informed of patient and operational concerns
- Assist with internal health screenings (e.g., TB tests, flu shots, drug testing)
- Support logistics for staff wellness initiatives
- Uphold HIPAA standards and patient confidentiality
- Follow all policies as outlined in the Personnel Manual and Employee Handbook
- Promptly report concerns related to care quality or staff conduct
- Strong organizational and communication skills
- Proficient with clinical software and general office systems
- Ability to work independently, prioritize tasks, and remain calm in high-pressure situations
- Familiarity with general office procedures (phones, faxing, copying, etc.)
- Must be able to stand, walk, bend, kneel, and occasionally lift up to 80 lbs
- Adequate vision, hearing, and manual dexterity required for clinical care
- Combination of office and patient home settings
- Exposure to varying home environments and infectious materials
- Moderate noise level and frequent interruptions
- Must be comfortable managing emotionally sensitive situations
