We work with people to help set and meet their personal goals!About the Job:The Diabetes Education Coordinator functions as the program coordinator for the Outpatient Diabetes Education Program. They provide clinical expertise for the education and management of persons with Pre-Diabetes, Type 1, Type 2 and Gestational Diabetes and for all age ranges – pediatrics, adults, and geriatrics. They provide clinical and administrative leadership in the ongoing development of the program, supports and communications organizational goals and decisions to the staff members.What You’ll Do:Conduct individualized needs assessment using both interview and self-assessment forms for all individuals/families and significant others enrolled in the outpatient diabetes education program for both newly diagnosed and those with long-standing diabetes.Provides individualized and/or group instruction in diabetes self-management to persons with diabetes and their families/significant others.The population served includes people with diabetes, prediabetes, polycystic ovarian syndrome, digestive health, lipid management, food allergies, weight management, failure to thrive, chronic kidney disease, and oncology.Who You Are:BS required, Masters degree preferred.Certification by the National Certification Board of Diabetes (CDE Member of the American Dietetic Association required.2-4 years’ experience in outpatient setting with patient teaching experience with some of it with patients with diabetes.
