
Referral Coordinator

- Interact with external partners, healthcare systems, and clients to ensure access to care for all SonderMind clients
- Facilitate care coordination activities such as lead and task management, referral processing, phones and post session documentation and reporting
- Assemble and maintain accurate and appropriate documentation for referred clients throughout their mental health journey
- Manage inbound and outbound calls and other forms of communication while applying standard guidelines to address or route messages to the appropriate care team member for follow-up when necessary
- Ensures all referrals are complete and are processed in a timely and accurate manner via EMRs and other referral sources (cases, faxes, email, phone)
- Utilize a variety of technologies to ensure the client is connected with the appropriate therapist and maintains the required support through an episode of care
- Conduct outreach to referred clients and partners to obtain critical information to aid in the journey to care
- Other responsibilities and ad-hoc projects from time to time based on business needs.
- Within two weeks, fully understand our referral journey and SonderMind's technology platforms
- Within one month, provide support for post session documentation
- Ongoing ability to adapt to the change in workflow and job tasks.
- Ongoing proactive identification of problems and asking questions to clarify and help solve problems.
- Ongoing communication, both written and verbal, to engage, learn, and take action
- Bachelor's degree or equivalent experience
- Experience in Care or Referral Coordination
- Strong desire to assist clients and provide support to those seeking mental health care
- Strong communication skills, both written and verbal
- Motivated and eager to learn, ability to adapt to new technologies, processes, and workflows
- Strong problem-solving skills and attention to detail
- Flexibility to work in a fast-paced, dynamic environment
