- Completing basic health and social needs assessments, digital documentation, and verbal and written communication for patients and their caregivers.
- Lived experience with diabetes and an understanding of the patient challenges is essential.
- Preferred: Completed Community Health Worker or Promotor/a training or certificate.
- Minimum: Valid California driver's license, reliable transportation, and insurance coverage as required by California state law.
- Preferred: CHW, Promotor/a, or medical assistant certification.
- Minimum: Competency, knowledge, and lived experience with a diabetes diagnosis and ability to connect with patients with diabetes. Ability to type and navigate on a computer.
- Preferred: Bilingual in Spanish and English. Ability to write, read, and speak in English for patient documentation.
- Minimum: Personal lived experience with a diabetes diagnosis.
- One (1) year of experience working in a community health or related service, including volunteer or internship positions in a community setting. Diabetes education or training.
