Summary;Responsible for the coordination of the MDS process, based on the comprehensive assessments of the resident's needs.Essential Duties and Responsibilities include the following. Other duties may be assigned.
Completes initial assessment of residents and all subsequent assessments.
Updates physician's orders as needed.
Responsible for entire MDS process.
Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.
Develops Comprehensive Care Plan with the Interdisciplinary Team.
Makes recommendations for charting requirements.
Informs staff of any changes regarding resident care and care issues.
Monitors "RUG" scores for optimal reimbursement.
May be required to take on-call needs and assist with routine LPN duties.

Must have a minimum of two years Long Term Care experience.
Certificates, Licenses, Registrations
Must be a Licensed Practical Nurse in the State of Ohio.
