- C N A license in Massachusetts active in good standing OR
- Certificate of recent graduation from MA Nurse Aide Training Course (DPH approved) & eligible to schedule C N A licensure exams (holds state certification within 120 days of hiring)
- Able to communicate in written and oral English. Comfortable with the resident population.
- Assists residents routinely and as needed with any and all activities of daily living, including oral hygiene, bathing, grooming, dressing, toileting, nutrition and feeding, exercise and mobility, transfers and ambulation, transportation and activity.
- Receives instructions and guidance from the licensed nurse(s) on the unit regarding individual resident needs.
- Consults resident 's written plan of care to obtain information and instructions regarding individual resident's needs.
- Performs assigned tasks according to established procedures and practices including:
- Gathering and recording data, e.g., vital signs, weight, intake and output, specimen collection, etc.
- Carrying out nursing routines, e.g., to prevent decubiti, contractures and immobility and to keep patients clean, comfortable, safe and secure, etc.
- Providing restorative nursing care, e.g., range of motion, positioning, preventative skin care, reinforcing teaching and providing assistance with activities of daily living, exercise and activity, etc.
- Assisting in maintaining the safety and cleanliness of the resident's total environment to include labeling and provision of personal care equipment and cleaning bedside table, bureaus and closets.
- Makes observations of resident's physical and mental state and records and reports observations to appropriate staff. Such physical and mental states to include: mental status, behaviour/mood, vital signs, weight, skin condition, mobility status, appetite/nutrition, activity level, level of independence in activities of daily living, and progress in restorative nursing programs.
- Attends and participates in at least 12 inservice and continuing education programs for growth and development per year.
- Performs other duties as directed.
