Title: Clinical Behavioral Health Care SpecialistLocation: Upper East Side; Brooklyn; Hamptons; Lower East Side; Midtown; Queens; West SideOrg Unit: Melissa Frey LabWork Days:Weekly Hours: 35.00Exemption Status: ExemptSalary Range: $85,200.00 - $93,100.00*As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC OfficesPosition SummaryServes as a licensed behavioral health clinician who works with a clinical team that advances behavioral health integration in the primary care setting using the principles of the Collaborative Care Model.Job Responsibilities
  • Provides evidence-based behavioral health interventions and psychoeducation.
  • Assesses and systematically collects and tracks data in a patient registry related to patient behavioral health status and works with the care team to develop the plan of care.
  • Participates in-person or virtual huddles when shared patients are discussed.
  • Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
  • Develops and maintains a working knowledge of community resources and managed care review processes.
  • Assumes responsibility and accountability for effectively managing behavioral health care of individual patients.
  • Documents history, assessments and interventions to reflect comprehensive and integrated approach to patient care. Ensures complete, accurate and timely documentation in the electronic medical record.
  • Identifies learning needs of the patient and families, provides patient education; educates by adapting to patient and family learning style utilizing the teach-back method.
  • Attends team meetings and case conferences to present and discuss patient disposition and outcomes data.
  • Assists with the coordination of care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools). Assists with management of urgent and emergent patient care situations.
  • Assists with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness.
  • Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
  • Performs other related duties as assigned and as appropriate to scope, education, and/or training.
Education
  • Master's Degree
ExperienceLicensed Clinical Social Worker (LCSW)/Licensed Master Social Worker (LMSW) or Licensed Mental Health Counselor (LMHC).Experience working with individuals and families affected by mental illness.Experience providing evidence-based psychotherapeutic approaches.Ability to thrive within a complex medical systemWorking understanding of the healthcare system.Knowledge, Skills and Abilities

Licenses and Certifications
