As our Client Care Manager, you’ll be at the heart of that mission: guiding families through one of life’s most emotional decisions with empathy, clarity, and expertise. You’ll help them imagine—and create—a personalized care experience that keeps their loved one home, connected, and thriving. Not institutionalized. Not reduced to a room number.
What You'll Do
- Collaborate with families to design thoughtful, personalized care plans tailored to each client’s evolving needs and preferences.
- Serve as the primary point of contact for families, referral partners, and care professionals—ensuring a seamless, supportive care journey.
- Regularly review and adjust care plans, helping our clients navigate changing circumstances while honoring what matters most to them.
- Cultivate meaningful relationships built on trust, compassion, and commitment to excellence.
- Partner with our team of care professionals to ensure plans are executed with the right level of attention, skill, and heart.
- You’re a natural advocate and guide—able to help families make confident, informed decisions during difficult times.
- You’re a collaborator who thrives on bringing teams together to deliver care that is both high-quality and deeply personal.
- You believe in proactive care, not just reactive responses.
- You understand that caregiving is about more than checklists—it’s about connection, respect, and joy.
- You bring emotional intelligence, clear communication, and a servant leader mindset to every interaction.

- You have a valid driver’s license and auto insurance.
- You have experience in the long-term care industry.
- A nursing license (RN or LPN) is a plus, but not required.

- Wait! Before You Apply, Do You Qualify? Make Sure...You have a thorough understanding of and at least 2 years of previous experience in the Senior Care industry.
- You have a valid driver's license & reliable vehicle with insurance.
- You possess strong leadership abilities.
- You hold a valid RN license in the state of IA. (Preferred)
- Your Schedule:Monday - Friday
- 8:00AM - 4:30PM
- Your Primary Responsibilities:Develop and implement personalized care plans for clients.
- Work closely with families, teammates, industry partners, and other stakeholders.
- Ensure holistic and quality care delivery to address the comprehensive needs of clients.
