- Provides psychosocial assessment / intervention to assist patients/families (Including clinically complex).
- Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides linkage to community resources to facilitate optimal care.
- Provides age specific care to patients/families in all age groups.
- Develops and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
- Maintains a current working knowledge of UH network services and promotes utilization of such services.
- Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
- Provides supervision to social work interns
- Performs other duties as assigned.
- Complies with all policies and standards.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- Master's Degree from accredited school of Counseling (Required)
- Hospital/Health Agency experience. (Preferred)
- Demonstrates ability to interact professionally with physicians, nurses and other members of the healthcare team. (Required proficiency)
- Demonstrates ability to apply theoretical and clinical knowledge in delivery of care. (Required proficiency)
- Multi tasks and prioritizes work. (Required proficiency)
- Performs autonomously as a professional. (Required proficiency)
- Excellent verbal and written communication. (Required proficiency)
- Licensed Professional Clinical Counselor (LPCC) (Required)
- LPCC-S (Preferred within 1 Year)
Must have an Ohio LPCC licensure. This position will see women's health patients, which will include underserved population, pregnant, antepartum, those suffering from depression, etc. Previous experience working with this patient population is strongly preferred.Physical Demands
