- Accurately administer and document all prescribed medications in accordance with the Medication Administration Record (MAR), following the seven rights of medication administration and physician orders in a timely manner, while ensuring resident's privacy, and dignity.
- Works in tandem with the nurse to ensure medications are reordered at least 7 days before they run out or as directed by the Health and Wellness Coordinator. The QMAP will assist the nurse daily in tracking delivery of medications to ensure they are delivered in a timely manner to ensure there are no missed medications.
- Immediately document administered medications in the resident's medical record
- Complete documentation of all incidents per Brookdale Policy before leaving shift while communicating with the nurse onsite or on call.
- Monitor residents for any changes in condition, adverse reactions or side effects and promptly report findings to the licensed nurse.
- Take and record vital signs as required before administering medications that may impact vital signs, based on Physician, Health and Wellness Director and Health and Wellness Coordinators, orders.
- Administer PRN (as needed) medications as prescribed by a physician.
- Report all medication errors to the Health and Wellness Director immediately.
- Collaborate with the care team to assist in the development and ongoing review of residents' care plans and report any changes in care needs.
- Identify and report any unsafe conditions that may pose a risk to residents' health or safety.
- Support residents' physical and emotional well-being, ensuring their safety.
- Provide Hands-On Care With Daily Living Activities Including Toileting, Bathing, Grooming, Dressing, Nail And Hair Care, Laundry, Mobility And Transfer Assistance, And Promptly Responding To Resident Calls.
- Based on state regulations and completion of required training/certification, Medication Aides/Techs will administer or assist with self-administration of medication and treatments as prescribed by the health care provider and observe/report responses to your supervisor.
- Certified Medication Aides/Techs make sure the medication supply room is organized and clean, assist with medication cart audits, and provide accurate counts of all medications. You will also communicate with pharmacies to coordinate medication delivery.
- Based on state regulation, completion of training/certification is required.
