Screen and assess families referred to Healthy Families for program eligibility, while identifying the presence of physical, mental, and emotional well-being factors and enhancing the home environment by providing resources to strengthen the parent-child relationship.Healthy Families Program Delivery (75%)
- Participate in clinical or case staffing for in-home counseling services to Healthy Families program participants, meeting established program outcome goals and upholding the CHS Standard of Care.
- Develop a set of actionable and targeted change management plans including communication, roadmaps, training, and resistance management plans.
- Directly transport, when necessary, or arrange for client transportation for activities such as medical appointments, counseling, parent leadership meetings, shopping, other social service agency meetings, or emergency services.
- Oversee compliance with HS/EHS, CHS, COA, licensing, contractual, quality, and outcome standards.
- Monitor quality of care by complying with contract requirements for contacting clients by phone, by letter, and/or by conducting home/residential visits.
- Collaborate with stakeholders, such as EBP developers, academic partners, provider agencies, community organizations, and cross-system partners to support EBP initiatives and implementation.
- Develop a set of actionable and targeted change management plans including communication, roadmaps, training, and resistance management plans.
- Provide access to health services and other community resources/referrals to support healthy childhood growth and development, as appropriate.
- Develop comprehensive client-driven Family Goal Plans that clearly identify specific goals, objectives, and service needs.
- Teach positive parenting skills and age-appropriate discipline options to assist in strengthening parent-child interaction to enhance the family functioning to improve self-sufficiency.
- Provide appropriate modeling techniques, education, activities, and other support as delineated in the Home Visit Service Plan.
- Promote the program at community resource fairs, meetings and other community events.
- Create and maintain client clinical and case records, documenting activities as required by the contract and CHS standards.
- Meet with supervisor weekly to provide comprehensive data updates to the Program Manager regarding the status of screening/assessment activity.
- Perform all job functions as set forth by the Healthy Families Florida contractual requirements to ensure the delivery of quality services.
- Maintain or exceed at least 80% of quarterly home visits as required by Healthy Families Florida.
- Develop a strong knowledge base and stay current on job-related issues and trends.
- Participate actively in departmental meetings, training and education, as well as the quality process.
- Comply with CHS’s code of conduct, policies, procedures and other obligations.
- Assist with training other team members and providing back up when necessary.
- Pick up projects on the fly; perform other duties as assigned from time to time.
- Demonstrate the CHS Common Bond values in the performance of all job duties.
- High school diploma or GED equivalent, required.
- Florida Driver’s License within 30 days from hire with daily access to a reliable and insured vehicle, required.
- Minimum of one year experience working with or providing services to children and families, required.
- Minimum of one year of experience working with culturally diverse young children and families in a human services or early childhood education setting, required.
