The Health Insurance Enrollment Project Coordinator supports the enrollment and case management of at-risk families in South Phoenix, ensuring they receive health insurance services and community resources. This role involves direct outreach, recruiting, educating, and assisting clients with health care enrollment while working closely with local health providers and community organizations.Key Responsibilities:
- Recruit and enroll families into health services.
- Provide referrals and assist with scheduling, prenatal care, and other social services.
- Conduct outreach in the community through events and informational materials.
- Ensure participants receive ongoing support through follow-ups, surveys, and coordination with partner agencies.
- Provide case management support and contribute to program quality assurance.
- Support clients in accessing essential healthcare and social services.
- Education: High school diploma or GED + 2 years’ experience in social services (WIC, Parents as Teachers, Head Start, or similar).
- Certifications: CPR/First Aid (to be completed within 6 months), Fingerprint/background check, Arizona driver’s license.
- Preferred: Bilingual (English/Spanish), experience in perinatal health care, and a certification in Public Health or Community Health Worker.
