We are seeking a Licensed Clinical Social Worker (LCSW) to join our collaborative healthcare team and make a meaningful impact on the lives of residents in skilled nursing and rehabilitation facilities throughout Florida. This is a unique opportunity to work alongside dedicated healthcare professionals in one of Florida's most respected healthcare organizations.Position Summary: As the LCSW, you will conduct initial evaluations, provide psychotherapy, and offer behavioral management to residents in long-term care and nursing facilities. Responsibilities:
Perform evaluations and ongoing therapy sessions
Provide evidence-based therapies such as CBT, ACT, and DBT
Collaborate with prescribers to manage patient care
Maintain comprehensive, timely documentation in EHR
Build and maintain strong therapeutic alliances with a diverse patient population
Travel to multiple facilities within your assigned territory
Required Qualifications:
Current LCSW
Active, unencumbered Georgia license
Proficiency in evidence-based therapeutic techniques (e.g., CBT, ACT, DBT)
Strong computer and technology skills
Current CPR certification
Valid Georgia driver's license and willingness to travel
Ability to multitask, prioritize, and make independent decisions within a collaborative team environment
Excellent communication and interpersonal skills
