SummaryPosition SummaryThe Education Specialist, or Licensed Dietitian, assesses and makes recommendations to meet the nutritional needs of patients, provides nutritional counseling and serves as a member of the patient care team. In addition to planning, designing, coordinating and evaluating educational programs in the St. Joseph's/Candler Health System, in disease management programs and in the community, to support the mission, vision, strategic direction and operational needs of the St. Joseph's/Candler Health System.Education

Bachelors in Nursing - RequitedExperience2-3 Years clinical experience - RequiredOne year experience working with a diabetic or special patient population.License & CertificationProfessional License with State of Practice - RequiredRegistration with Commission on Dietetic Registration - RequiredCertified Diabetes Care and Education Specialist with the American Association of Diabetes Educators or a National Certification in Special Populations is preferred within 12 months of hire date.Core Job Functions
  • Designs, develops, updates and evaluates educational curriculums. Accreditation standards and principles are incorporated in educational planning. Participates on committees and serves as a content expert when applicable.
  • Collects and reviews information on lifestyle. Educates based on health risks and needs, sets realistic goals and reviews as needed. Researches materials to stay current on treatments of chronic disease.
  • Develops the plan to attain the mutually defined goals to achieve desired outcomes. The plan integrates current care practices and established principles of teaching and learning. The plan is coordinated among the healthcare team members, the client, his or her family, significant others, and other relevant support systems, and the referring provider.
Nutrition education is given at the time appropriate for optimal learning for the patient and family using approved educational materials and tools individualized and used to support the education. Learning needs are assessed and education modified to meet the needs of the patient and family. Education is documented in the appropriate section of the electronic medical record.Monitors and evaluates effectiveness of programs thru data collection. Makes recommendations for changes based on outcomes.
