Overview:This position provides general medical assessment, evaluation and treatment of patients referred to palliative care consultative services in the clinic, home and hospital.   • Join an excellent team of non-invasive, interventional, and EP cardiologists, along with Cardiovascular Surgeons and a vascular surgeon.
  • Our team also includes several other APPs, a strong RN staff, and other support staff.
  • Fantastic working schedule of Monday – Friday 8:00am-4:30pm
  • No Holidays, No Call and No Weekends!
  • Coverage will include: hospital rounding and main clinic
  • Responsibilities to include: following up on patients, new consults and discharging weekly
  • Enjoy a high level of independence
  • Outstanding referral system from primary care
Qualifications:
  • Ability to work well independently and under pressure.
  • Strong communication/interpretive and analytical skills.
  • A high level of independent judgment and ability to handle empowerment for independent action.
  • Ability to pass the Wisconsin Caregiver Background Check and/or Illinois Health Care Worker's Background Check.
Education/Licensure:
  • Graduate of an accredited MSN program
  • Current IL RN license
  • Current APNP
  • Current DEA number
  • ATLS certification
  • Other certifications as required
