Position Summary:Provides services to clients in accordance with the State Nurse Practice Act, Agency policy, and accepted professional standards of practice.Delivery of patient care as appropriate to provide services in accordance with Agency policies under the direction of the Registered Nurse.Qualifications:
  • Graduate of an accredited school of professional nursing.
  • Current license as a Licensed Practical Nurse in the state of Connecticut.
  • Minimum of one (1) year experience in home health setting or equivalent experience preferred.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Strong communication and interpersonal skills.
Essential Functions:
  • Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
  • Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes). Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:
  • Assessment skills as applied to the client, family/support system, and environment
  • Teaching skills according to the client's needs and within the context of the client's environment, value system, and physical/emotional status
  • Medication administrations
  • Practices nursing with respect for individual, cultural, and spiritual differences.
  • Communicates effectively and in accordance with Agency requirements.
  • Reports significant changes in client status to physician and other members of the team in a timely manner consistent with client needs
  • Maintains client records showing systematic assessment, planning intervention, and evaluation
  • Participates in team meetings and clarifies individualized reporting responsibilities with all new assignments
  • Documents legibly and according to Agency documentation guidelines and standards
  • Follows Agency guidelines for completion of forms, reflecting the orders and care plan. Submits forms within 48 hours including all communication and coordination
  • Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in-home care situations. Initiates referrals as needed to other health team members when delegated by the Case Manager.
  • Maintains confidentiality in all aspects of the job.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Demonstrates knowledge of safety/infection control practices by complying with established policies and procedures
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Demonstrates safe practice in the use of equipment
  • Assesses environmental safety and takes initiative to prevent accidents and promote safety
  • Notifies supervisor of need for review and/or training related to equipment, procedures, or safety/infection control practices
  • Participates in ongoing, professional self-development.
  • Participates in all mandatory education per Agency guidelines
  • Identifies needs for personal professional growth
  • Demonstrates competency in areas of assignment or identifies the need for additional training
  • Performs other duties as assigned.
  • Provides nursing care under the supervision of a Registered Nurse and according to the plan of care derived from physician's orders and Registered Nurse assessment. Nursing Care includes physical care, emotional support, and patient and family education.
  • Recognizes and reports symptoms that indicate changes in the condition of the patient to the Case Manager.
  • Provides nursing care and services that is in accordance with Agency patient care and general medical policies.
  • Assists the physician and/or Registered Nurse in the performance of specific procedures require by the patient and at an appropriate level for the LPN.
  • Adheres to strict aseptic technique when performing treatments requiring its use.
  • Provides patients with the knowledge and assistance to learn appropriate self care techniques.
  • Accurately records patient's condition in the Nurses notes.
  • Observes, records and reports to the appropriate professional the symptoms, reaction and changes including:
  • General physical and mental condition of patients, and signs and symptoms which may be indicative of untoward changes; and
  • stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
  • Performs selected nursing procedures such as:
  • Administration of medications and treatments prescribed by the physician: and
  • preparation and care of patients receiving specialized treatments administered by the physician or the professional nurse.
  • Assists with the rehabilitation of patients according to the medical care plan through being aware of and encouraging the interests and special aptitudes of patients; encouraging patients to help themselves within their own capabilities in performing activities of daily living; knowledge and application of the principles of prevention of deformities, the normal range of motion, body mechanics, and body alignment.
  • Promotes cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences when appropriate.
  • Administers medications and treatments that are assigned by the Registered Nurse and/or Nursing Supervisor listed on the plan of care, and records these with accuracy and completeness.
  • Maintains the patient's equipment and room as necessary for patient care and safety.
  • Participates in staff In-service programs. (Must attend 12 hours annually).
  • Participates in regular performance evaluations with Case Manager.
  • Maintains a continuing knowledge of competencies related to the nursing profession by participation in In-service and educational programs, conferences, workshops and professional organizations.
  • Understands human relationships between and among patients and personnel, patients and families, families and personnel; recognizes and understands cultural backgrounds and spiritual needs, respects the religious beliefs of individual patients.
  • Abides by administrative policies regarding nursing coverage and responsibilities, scheduling availability, and provides support to the Nursing Department as deemed appropriate.
  • When necessary, works on additional projects within scope of knowledge, as requested by Case Manager/Clinical Care Supervisor.
Physical/Environmental Demands:Requires the ability to sit, stand, pivot, twist and change position frequently Requires the ability to lift, push, pull, and carry 20-50 lbs frequently.Requires the ability to climb stairs, navigate all types of residences and access means.Requires the ability to utilize telephone, laptop computer and clinical equipment.Requires the ability to travel to/from Agency, client homes and other professional appointments on a frequent daily basis.
