- Training: Training will be held at Atlanta GA 30339 (Training) approximately a week of training

- Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
- Connecting with community partners and/or organizations to build partnerships that help to address the needs of our members.
- Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
- Team/Individual is measured on the number of successful home visits completed per month (60% of the home visits will be successful).
- Staff will be assigned to the Preceptor within the department and will go over the role and responsibilities of the position, attendance policy, expense reimbursement, etc.
- Assigned mandatory compliance e-learning trainings through Client's University (self-directed).
- New Hire Training.
- Key Skills; Community engagement, Managed Care & Microsoft Office Suite.
- Knowledge of Community Health Practices:
- This includes understanding the health needs of the community, local health disparities, preventive health strategies, and social determinants of health.
- A strong foundation in these areas allows a CHW to effectively serve the community.
- Data Collection and Reporting:
- Ability to gather health-related data through surveys, interviews, or other methods and report on this data accurately.
- Familiarity with digital health tools, record-keeping systems, or data analysis can be crucial for tracking community health outcomes.
- Cultural Competency and Language Proficiency:
- Given that CHWs often work with diverse populations, understanding the cultural context and being able to communicate in the languages spoken by the community is a must.
- This could include fluency in specific languages or an understanding of cultural norms that affect health behaviors.
- Must have Reliable Transportation/Willingness to travel, travel will be 80% of the role. Travel will entail the entire state of GA.
- Service Coordination Skills (knowledge of community resources and partners within designated region).
- Strong Communication Skills - verbal and written.
- Strong Organizational Skills / Time management.
- Bilingual.
- Community Health Worker Training/Certification.
- Managed Care and/or medical experience.
- 2+ years of experience with community outreach.
- High school diploma or equivalent.
- Microsoft Office Suite (Power Point, Word, Excel) - Intermediate.
- Valid driver's license and proof insurance
