
Substance Use Counselor

- Conduct individual assessments and evaluations to determine service eligibility.
- Complete Bio-psychosocial assessments to create personalized treatment plans.
- Facilitate intake processes and ensure smooth patient onboarding.
- Develop and review individual treatment plans tailored to each patient’s needs.
- Provide individual and group counseling services at regularly scheduled intervals.
- Create aftercare and discharge plans to support long-term recovery.
- Collaborate with physicians, nurses, and fellow counselors to optimize patient care.
- Maintain accurate patient records and documentation in compliance with state and federal guidelines.
- Required Licenses: CAC I, CAC II, LGPC or LPC
- Experience: At least 1 year of counseling or internship experience in addiction treatment
- Preferred Background: Experience working with chemically dependent individuals
- Skills: Positive, non-judgmental, and compassionate attitude towards patients
- Compliance: Ability to pass a drug screen and criminal background check
