We're a leading long-term care and rehabilitation facility looking for a compassionate Licensed Practical Nurse to join our team. If you're ready to provide high-quality care in a supportive, patient-centered environment, we want to talk to you.Your Role: As a key member of our interdisciplinary team, you'll work closely with RNs, physicians, and rehabilitation specialists to ensure our residents receive the best possible care. Your responsibilities will include: * Medication Administration: Accurately administer medications and treatments.
  • Patient Monitoring: Monitor vital signs and health status, and report any changes to the supervising RN or physician.
  • Rehabilitation Support: Collaborate with physical, occupational, and speech therapists to support residents' recovery goals.
  • Documentation: Maintain accurate and timely records in accordance with all regulations.
  • Patient Care: Assist with activities of daily living and provide emotional support to residents and their families.
Qualifications: * Current and valid LPN license.
  • Strong clinical skills, including medication administration and wound care.
  • Excellent communication and teamwork skills.
  • BLS/CPR certification.
