Job Title: Community Health Assistant
Location: Phoenix, AZ
Duration: 6 MonthsPosition OverviewThe Community Health Assistant will provide outreach, education, and support services to at-risk families and hard-to-reach populations in the South Phoenix area. This role includes recruitment and enrollment into Healthy Start services, assisting case managers, conducting health and safety questionnaires, and connecting families with community resources. The position is focused on supporting prenatal health, infant wellness, and family stability.Essential Job Tasks(This is not an all-inclusive list; additional duties may be assigned as needed)Conduct outreach to recruit and enroll eligible families into Healthy Start services and assist with scheduling case management appointmentsProvide support to families by offering community referrals, transporting clients to prenatal appointments, and encouraging medical, social, or economic assistance as neededCollaborate with participants, partner agencies, and providers to ensure health and social service needs are being met; develop plans to address unmet needsConduct follow-up with families by phone, mail, or email to confirm services have been receivedParticipate in community outreach and education efforts, including distributing brochures, speaking with residents, and giving presentations to raise awareness about prenatal care and infant healthServe as non-medical support during labor when needed (similar to a doula role)Enter and maintain accurate client data for case documentation, follow-up reports, and program monitoringParticipate in quality assurance and case management meetings, presenting cases for review and discussionCollect participant feedback and make recommendations for improving program servicesQualificationsHigh school diploma or GED requiredTwo (2) years of experience working with pregnant women, infants, or toddlers in a social service setting (e.g., WIC, Head Start, home visiting programs, or similar)Post-secondary education may substitute for required experience on a year-for-year basisCPR and First Aid (adult and infant) certification required within six months of hireMust pass fingerprinting and background checkMust possess or be able to obtain a valid Arizona driver’s license and Vehicle Use PermitKnowledge, Skills, and AbilitiesKnowledge of:Community health and perinatal health issuesLocal healthcare and social service resourcesSkill in:Building rapport with diverse families and community membersClear verbal and written communicationAdapting communication style to meet client needsAbility to:Use computer equipment and software for documentation, reporting, and communicationProvide culturally appropriate health educationWork effectively with clients, coworkers, and community partnersMaintain reliable attendancePreferred Education & ExperienceSix (6) months of experience in perinatal healthcareOne or more years of college coursework completedBilingual (English/Spanish) strongly preferredPreferred Certifications & TrainingCertification as a Public Health or Community Health Worker from an accredited program OR enrollment in such a program with eligibility to be certified within six (6) months of hireWorking ConditionsMay be required to work some evenings and weekends for outreach eventsMust participate in emergency response training and be available in the event of a public health emergencyMust provide proof of required immunizations (MMR, Tdap, Varicella, annual Influenza; Hepatitis B and/or TB testing may also be required) within one month of hireFor more details reach at vgill@navitassols.com or Call / Text at 516-862-1203.
