Overview:UChicago Medicine AdventHealth Bolingbrook, GlenOaks, Hinsdale and La Grange hospitals are affiliated with UChicago Medicine, one of the top academic medical centers in the country. We are also part of AdventHealth, a unified health system with over 50 hospital campuses and hundreds of care sites in diverse markets throughout 9 states. Responsibilities:UChicago Medicine AdventHealth is hiring for our House Calls program. This is a growing program which is so needed in our regional communities. This program serves our homebound patients, giving them access to primary care and chronic condition management in the comfort of their own homes.
  • Physicians with experience in home care a plus, but not required
  • Weekly schedule is generally Monday – Friday, with occasional Saturday visits
  • Must be able to be flexible with your hours to help us meet the scheduling needs of our patients
  • Patient visits scheduled to keep you in one area each day, if possible, to reduce travel time
  • Resources available for lab work, imaging, procedures, etc.
  • Work collaboratively with APPs, Social Workers and Palliative Care
  • Excellent Benefits, PTO and CME
  • Occurrence-based malpractice insurance, includes tail coverage
  • Annual Salary: $220K-$280K
Qualifications:REQUIRED QUALIFICATIONS:Education/Training
  • Graduate of an accredited medical school and US residency program
  • Current Illinois Physician medical license in good standing
  • Board Certified/Eligible, required
