Key Responsibilities:Participates in the Admission Process
Includes preadmission activities and/or completing admission documents to ensure smooth onboarding for individuals served.Provides Psychoeducational Groups and Case Management
Facilitates group sessions and delivers individualized case management activities to address client needs.Coordinates Care and Maintains Treatment Team Involvement
Ensures comprehensive care through active coordination and participation in treatment team meetings.Ensures Accurate Documentation
Appropriately documents all services provided in the medical record to maintain compliance and continuity of care.Maintains Professional and Culturally Competent Relationships
Builds and fosters relationships with stakeholders while demonstrating age-specific and cultural competency in all aspects of work.
Qualifications & Requirements:Education:
- Required: High School Diploma or equivalent.
- Preferred: Bachelor’s degree in social work, psychology, or related field from an accredited college or university.
- Preferred: Two (2) years' experience.
