This is a part time position in a health care facility open 8 hours a day, Monday through Friday except for holidays recognized by the Hemophilia Outreach Center. Expected hours for this opening are 8:00am until 4:30pm with rotational call. One scheduled shift per week with additional hours on an as needed basis depending on schedule of events, operational demand, and/or patient demand.Position Purpose:
The Nurse Coordinator provides quality nursing care and education to clients and families of the Hemophilia Outreach Center as well as educate the surrounding community on bleeding disorders.Education/Criteria:
Graduate of accredited nursing program with Associate Degree, or BSN. The Nurse Coordinator must be licensed as a Registered Nurse by the State of Wisconsin or reciprocal state. Must have and maintain CPR for healthcare provider certification.Preferred Knowledge, Skills, and Abilities:
- At least 3 years of general nursing experience required
- At least 1 year clinic experience preferred
- Phlebotomy and venipuncture experience, preferably expertise, necessary
Needs to be self-motivated and able to complete work independently. Also needs to work cooperatively with others, including staff physicians, in a multi-disciplinary team environment. Strong organizational and professional communication skills are necessary.Responsibilities/Duties:
Patient Care
- Nursing assessments including physical assessment skills
- Telephone triage of patients
- Treatment planning: writing, implementing, maintaining, updating
- Coordination of Care: acts as a liaison between the Center, physicians, hospitals and other outside sources. Communicates and advocates in the best interest of the patient.
- Clinic services: Provides patient care on a scheduled and walk-in basis, prepares and coordinates comprehensive clinic with other team members
- Phlebotomy, venipuncture and infusions
- On-call care
- Some pharmacy-type duties including the dispense and administration of bleeding disorder medications.
- Ability to chart thoroughly
- Assist with clinical studies including blood draws, study forms and data entry
- Provide education regarding bleeding disorders to patients, families and community members including medical staff, schools and daycares as needed/requested
