- Complete diagnostic assessments
- Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in health and behavior change
- Support the team with the day-to-day management of complex patients
- Provide brief evidence-based direct care to patients
- Engage in population health activities to support risk stratified populations
- Required to complete system credentialing to perform pro fee billing
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
- Partners with patient care giver in care/decision making.
- Communicates in a respective manner.
- Ensures a safe, secure environment.
- Individualizes plan of care to meet patient needs.
- Modifies clinical interventions based on population served.
- Provides patient education based on as assessment of learning needs of patient/care giver.
- · Fulfills all organizational requirements
- Completes all required learning relevant to the role
- Complies with and maintains knowledge of all relevant laws, regulation, policies, procedures, and standards.
- · Fosters a culture of improvement, efficiency, and innovative thinking.
- · Performs other duties as assigned
- Master’s or doctoral level degree in counseling, social work or psychology
- 2 years experience post licensure
- One of the following licenses
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Professional Clinical Counselor (LPCC) or
- Licensed Independent Clinical Social Worker (LICSW) or
- Licensed Psychologist (LP)
- Licensed Graduate in Social Work will be considered under certain circumstances
- 5 years experience post licensure
