Case ManagementFirst Shift | 32-40 HoursRotating WeekendsBenefits!Day Kimball Healthcare is Hiring Case ManagementJob Summary for Case Management:Identifies case management needs in collaboration with the patient, significant others, payors and healthcare team. Assesses needs, plans, implements and evaluates post-discharge needs to meet patient needs along the continuum. Manages resources to provide the highest quality, most cost-effective care in the most appropriate setting. Negotiates, procures, monitors and coordinates services and resources throughout an episode of care. Plans care to maximize patient's quality of life. Assists with transfers to other acute care/rehab facilities. Assists with scheduling of outpatient services. Performs utilization review for appropriate documentation and level of care criteria.Essential Responsibilities for Case Management:Assesses clinical and psycho-social aftercare needs of patients and significant others; plans, coordinates and makes appropriate referrals. Evaluates the effectiveness of the plan by collecting data on measurable outcomes. Modifies the plan accordingly. Advocates for patients and significant others, helps them negotiate the healthcare system.Performs utilization management functions to assure appropriateness of care, level of care, length of stay and compliant documentation. Communicates with payor representatives to foster continuity of care and the highest quality care at the most appropriate setting.Requirements for Case Management:
  • Graduate of an accredited school of nursing required (BSN preferred).
  • Current CT State license required.
  • CCM preferred.
Experience for Case Management:
  • Minimum of three years nursing experience in either a hospital or homecare setting required.
  • Membership and participation in professional organizations preferred.
  • Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire required.
