Full job descriptionWe are seeking a licensed Physical Therapist (PT) to join our team as a Documentation Reviewer. In this role, you will review, analyze, and provide feedback on patient documentation to ensure compliance with payer guidelines, improve chart accuracy, and support clinical quality.This is an excellent opportunity for PTs looking for a flexible, non-clinical role where you can leverage your clinical expertise without providing direct patient care.Responsibilities
  • Review physical therapy documentation, including evaluations, treatment notes, and progress reports.
  • Ensure charts meet Medicare, commercial payer, and compliance standards.
  • Identify gaps in documentation and provide constructive feedback to improve quality and support medical necessity.
  • Assist in maintaining consistency and accuracy across patient records.
  • Collaborate with clinic staff to improve EMR workflows and documentation efficiency.
  • Stay current with industry regulations and payer requirements.
⸻Qualifications
  • Active Physical Therapist (PT) license in any U.S. state.
  • Minimum 2 years of clinical experience in physical therapy.
  • Strong understanding of Medicare, commercial payer guidelines, and medical necessity documentation.
  • Prior experience with chart audits, utilization review, or documentation compliance preferred.
  • Familiarity with EMR systems.
  • Excellent attention to detail, time management, and communication skills.
