- Review physical therapy documentation, including evaluations, treatment notes, and progress reports.
- Ensure charts meet Medicare, commercial payer, and compliance standards.
- Identify gaps in documentation and provide constructive feedback to improve quality and support medical necessity.
- Assist in maintaining consistency and accuracy across patient records.
- Collaborate with clinic staff to improve EMR workflows and documentation efficiency.
- Stay current with industry regulations and payer requirements.
- Active Physical Therapist (PT) license in any U.S. state.
- Minimum 2 years of clinical experience in physical therapy.
- Strong understanding of Medicare, commercial payer guidelines, and medical necessity documentation.
- Prior experience with chart audits, utilization review, or documentation compliance preferred.
- Familiarity with EMR systems.
- Excellent attention to detail, time management, and communication skills.
