No Holidays, No WeekendsGet MORE from your career with Hartford HealthCareHartford HealthCare, the largest and most comprehensive healthcare system in Connecticut, has a new growth opportunity for APP Diabetes Education Program at The Hospital of Central Connecticut in New Britain, CT.Under the clinical supervision of the Endocrinology Medical Director and the general supervision of the Administrative Director, provides medical consultation and follow up for Diabetes and endocrine care in the outpatient setting.Performs assessments and treatment according to established practice goals and philosophy of the Department of Medicine in collaboration with the hospitalist and/or primary care physician of record or their designee.Some highlights of this opportunity include:· Provides direct patient care in office consultations, including assessment, work-ups, triage, and other patient care activities and documents in EMR system (EPIC).· Obtains and documents health history, performs and documents physical exam, and orders and interprets diagnostic tests· Provides counseling to patients regarding diagnosis and expected course of treatment· Collaborates with physicians to provide appropriate direct care interventions within scope of practice· Collects, organizes, records, and discusses data as appropriate and uses sound clinical judgment based in best practice standards and evidenced based medicine· Prescribes pharmaceuticals according to patient needs and conducts appropriate patient follow-up & outreachShift – Part Time – 24 hoursAdvanced Practitioners EnjoyAt Hartford HealthCare newly graduated and experienced providers are provided with the support and mentorship they need to succeed through on the job, classroom, and procedural training.
