Location: Hagerstown MD 21740 Field role, across Washington County, MD
Home Health Physical Therapist | Summary and Highlights:The Home Health Physical Therapist will provide home health services across Washington County, MD. Also oversee PTA (Physical Therapist Assistant) services throughout the region.
This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Home Health Physical Therapist | Responsibilities:
Perform in-home physical therapy evaluations and treatments based on physician referrals/orders.
Conduct comprehensive assessments using appropriate tools and methods.
Develop, implement, and regularly reassess individualized treatment plans and patient goals.
Supervise Physical Therapist Assistants and Aides in accordance with state practice acts.
Complete OASIS assessments and documentation accurately and on time.
Maintain ongoing communication with patients, families, and healthcare team members.
Recommend and assist in obtaining adaptive equipment to enhance patient care.
Document evaluations, treatments, and billing in compliance with regulations and standards.
Home Health Physical Therapist | Requirements:
Education: High school diploma or equivalent.
Attendance: Commitment to mandatory attendance during the first 90 days.
Licensed Physical Therapist in the state of Maryland (required)
Previous home health experience preferred
Experience with HomeCare HomeBase software is a plus
Strong clinical judgment, time management, and communication skills
Must be able to travel throughout Washington County, MD for home visits
