Provides day-to-day coordination of outpatient services and is accountable to clinic administration. The individual will support patient safety, quality improvement, patient satisfaction, scheduling and supply management.SUPERVISION: Directly supervised by the Director of Nursing.TYPICAL PHYSICAL DEMANDS: Requires standing, walking, stooping, bending, and kneeling. Ability to lift up to 50 pounds maximum with frequent lifting and/or carrying objects weighing up to 25 pounds. Requires the use of office equipment, i.e., copiers, computers, etc., and medical equipment, i.e., accuchecks, EKG, Holters, etc.Duties:
  • Administers medications, immunizations, and treatment and observes individuals for unusual symptoms and reactions.
  • Conducts physical assessments of individual health needs and implements appropriate action.
  • Implements and monitors case management service plans.
  • Instructs and assists individuals in meeting their health care needs.
  • Prepares, maintains, and reviews individual records and individual assessments. May participate in quality assurance reviews.
  • Complies with all State/Federal/Local standards; Facility Policy, ICF Standards, and Texas Nurse Practice Act.
  • Provides care in collaboration with other nursing staff members
  • Demonstrates knowledge of and participates in Improving Organizational Performance activities.
  • Acquires and maintains knowledge in nursing practice and assumes responsibility for the professional development of self, other nursing staff and students.
  • Contributes to and supports the effective management of the department/clinic.
  • Incorporates standards of practice, clinical practice protocols, etc. into practice in order to maximize clinical effectiveness.
  • Demonstrates sensitivity to the values of self and others.
  • Integrates personal values with the Center's Mission
  • Involves patient/family in plan of care incorporating their cultural, spiritual and other belief systems.
  • Identifies recurring clinical practice issues and contributes to the development of specific plans with the Chief of Clinical Services to relay at Clinical Leadership and/or the Compliance, Performance Improvement and Risk Management (CPIRM) meetings.
  • Performs other duties as assigned.
Qualifications:
  • Graduate of an accredited LVN program.
  • Active license as a LVN by the Texas State Board of Registered Nurse Examiners.
  • Minimum of two (2) years of nursing experience is preferred.
  • BLS Certificate is required.
  • Bilingual in English and Spanish is preferred.
  • Must be able to travel between clinic locations in Webb County, Zapata County and Jim Hogg County.
