
Nurse Clinician - Discharge Coordinator - Stead Family Children's Hospital NICU (80%)

- Coordinate and/or assist in the planning process for discharge of our NICU infants
- Serve as a resource and liaison for bedside nurses, patients and families for Infants being discharged from NICU
- Provide follow-up phone calls to families of children post discharge from our NICU
- Assist with discharge planning including providing education to patients and families to provide a smooth transition from hospital to home
- Discharge process: ensuring discharge orders are complete, reviewing discharge summary and AVS for appropriate HCI, provide teach back with patients/families, monitoring prescription fill time to pharmacy, assist with obtaining necessary supplies upon discharge if needed., review discharge instructions with patients, ensure follow-up appointments are scheduled when possible and are accurate on AVS, provide patients the clinic and emergency contact information on discharge
- Participating and/or lead daily unit bed huddles in coordination with the multidisciplinary team to manage care and review patients’ treatment plan with a focus on discharge planning. (why are they here, what are the goals to discharge and what needs to be done to achieve discharge)
- Meet with patients to discuss overview of discharge plan, equipment needs and education. Anticipate and evaluate any transportation needs, etc. for discharge providing patients/families the EDOD for planning purposes
- Document interactions with patients and families regarding discharge planning, discharge barriers and interventions completed
- Assist with contacting patients 48-72 hours after discharge to follow-up on any post hospitalization questions and concerns they may have
- Ensure appropriateness, timeliness and completion of medical and ancillary services
- Discuss patient priorities with therapies (PT, OT, ST) to ensure patients are being seen in a timely manner to establish needs upon discharge
- Collaborate with physicians regarding patient population care needs, discharge planning and providing length of stay allocations
- Contact patient procedure/testing areas to facilitate early testing times for patients with potential to discharge in near future
- Collaborate with unit nursing staff and social worker on treatment plans, discharge barriers and family issues that may affect discharge
- Assist Utilization Management nurses in obtaining appropriate patient information for their continued stay reviews as needed
- Communicate any length of stay barriers that are unable to be solved to supervisor for further review
Schedule: 20-40 hours per week, M-F, 0730-1600

- A Baccalaureate degree in Nursing
- Reasonable (1-3 years) recent NICU nursing experience
- Current license to practice nursing in Iowa
- Demonstrates expertise with discharge planning for NICU patients with complex health care needs (involving multiple departments and external agencies).
- Demonstrated expertise in specific clinical area of practice
- Proficiency in computer applications including Microsoft Excel, Word and PowerPoint
- Excellent written and verbal communication skills
- Demonstrated excellent organizational skills, critical thinking, and problem-solving skills
- Experience working with an interdisciplinary team
- Previous case management experience is desired and/or strong discharge experience of the NICU patient population
- Certification in Neonatal nursing is desired
