- Care Coordination: Coordinate patient care across the continuum for Hem/Onc and BMT patients, from pre-admission, inpatient stay, discharge, and transitioning to outpatient services.
- Multidisciplinary Collaboration: Collaborate with RNCCs, attending physicians, and interdisciplinary teams to ensure proper clinical protocols, treatment plans, and manage complex social, medical, or logistical needs.
- Insurance and Authorization: Ensure timely insurance benefit checks, prior authorizations for medications, home infusions, and necessary procedures are completed.
- Daily Rounds and Discharge Planning: Participate in daily rounds, assist with discharge planning, and ensure completion of all protocol-required evaluations.
- Chemotherapy & BMT Coordination: Support chemotherapy admissions and coordinate care, including chemo schedules, diagnostic testing, and notifying necessary team members (pharmacy, attending, etc.). Assist with transplant evaluations, scheduling, and pre/post-transplant care.
- Anticoagulation Management: Collaborate with the anticoagulation clinic to manage outpatient needs such as supplies, follow-ups, and patient/family education on anticoagulant therapy.
- Patient Education: Educate patients and caregivers on Hem/Onc processes, central line care, medication administration, signs of complications, and pre/post-transplant care. This includes educating families on bone marrow transplant protocols and aftercare.
- Communication: Maintain clear communication between inpatient and outpatient teams, home care agencies, and specialty clinics regarding changes in patient status, discharge needs, and follow-up care.
- Post-Discharge Coordination: Coordinate post-discharge care, including follow-up appointments, home infusion needs, specialty equipment, and necessary therapies.
- Transport and Inter-Facility Coordination: Assist in arranging transportation for patients as needed and facilitate inter-facility coordination for specialized care.
- Documentation: Ensure accurate and timely documentation of care coordination activities, patient assessments, and educational materials in the EMR.
- Provide care that is adjusted to the developmental level and cultural differences of patients.
- Engage patients and families in shared decision-making.
- Communicate effectively and respectfully with patients, families, and the healthcare team.
- Ensure patient safety and maintain confidentiality.
- Participate in performance improvement initiatives related to Hem/Onc and BMT care.
- Proficiently use EMR systems (Epic, Beacon) for documenting care coordination, assessments, and educational interventions.
- 3+ years of clinical nursing experience in oncology, hematology, or transplant care, including care coordination or pre/post-transplant experience.
- MN Registered Nurse (RN) License
- Basic Life Support (American Heart Assoc or Red Cross)
- OCN (Oncology Certified Nurse) or BMTCN (Blood and Marrow Transplant Certified Nurse)
