- Variety of in-home and assisted living assignments, procedures, and treatments
- Mobile time tracking and care notes, employees can easily clock in and out for work right from a mobile device
- Assist the client with toileting activities, including bowel and bladder care.
- Must consistently cover shifts that are specified above
- Must demonstrate flexibility in covering other shifts on short notice
- Hoyer lift/Sit-to-stand lift
- Bathing
- Grooming, including oral care
- Assist the client with dressing and undressing
- Assist in feeding the client, if necessary
- Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively encourage movement)
- Assist client with active and passive range of motions as defined by the plan of care
- Provide client with help walking and transferring
- Observe client’s physical, mental and emotional conditions.
- Report any changes to the Director of Nursing or supervisor
- Document observed changes
- Assist client in the self-administration of medication (medication reminder)
- Perform light housekeeping
- Perform meal preparation and clean up
- Change bed linens and make up the client’s bed
- Maintain an orderly client environment
- Follow principles of infection control and Universal Precautions
- Provide transport to doctor appointments, outside activities and shopping
- Over 18 years of age
- Licensed driver with reliable transportation
- Auto insurance in accordance with the organization’s requirements if transporting clients
- Minimum of one year of documented experience preferred
- Self-directing with the ability to work with little direct supervision after initial training
- Empathy for the needs of the client
- Ability to read, write, speak and understand English and communicate effectively
- Treats clients, staff and the public with courtesy, respect and presents a positive public image
- Works as a team member
- Availability to work weekends is a plus (weekend incentive offered)
- Ensures confidentiality and security of the client’s medical information (HIPAA)
- Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
