Reports to: Clinical DirectorPosition Overview:We are seeking a compassionate and dedicated On-Call After-Hours Hospice Nurse to provide high-quality, compassionate care to patients in the comfort of their homes during after-hours shifts. The ideal candidate will be experienced in palliative care, have strong clinical skills, and be capable of working independently to manage patient symptoms and provide support to families during end-of-life care.Key Responsibilities:Provide Direct Patient Care: Deliver professional nursing care to hospice patients during after-hours, including symptom management, pain relief, and support for comfort care.Assess Patient Conditions: Conduct thorough assessments of patient conditions, including vital signs, pain levels, and emotional well-being, to ensure appropriate interventions.Medication Administration: Administer prescribed medications, including pain management medications, as needed, and educate patients and families on their use.Emergency Response: Respond to urgent patient needs, including acute symptom management, and provide crisis intervention in line with hospice care protocols.Family Support: Offer emotional support and education to families, guiding them through the hospice process, and helping them understand the patient's care plan.Documentation: Accurately document patient interactions, assessments, and any changes in condition in the patient's medical record, ensuring continuity of care.Collaboration: Communicate effectively with the hospice care team, including physicians, social workers, chaplains, and other healthcare professionals to ensure the patient's holistic needs are met.On-Call Availability: Be available for emergency calls and patient visits during evenings, nights, weekends, and holidays, as required by the on-call schedule.Qualifications:Education: Registered Nurse (RN) with a valid license in Illinois.Experience: Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care preferred.Skills:Strong clinical assessment and critical thinking skills.Ability to work independently and manage a caseload in a home setting.Excellent communication and interpersonal skills to interact with patients, families, and healthcare team members.Comfort with end-of-life issues and delivering compassionate care in sensitive situations.Certifications: BLS (Basic Life Support) certification required; Hospice and Palliative Care certification preferred.Physical Requirements: Ability to lift, carry, and move patients as necessary and manage the physical demands of home healthcare.Additional Requirements:Must have reliable transportation and be willing to travel to patients' homes within the service area.Flexible schedule and availability for on-call shifts, including nights, weekends, and holidays.Strong organizational skills and attention to detail.Must pass a background check and drug screening.Why Join Us:Impact: Be part of a compassionate, supportive team providing exceptional care to patients and families during a critical time.Autonomy: Opportunity to work independently and make meaningful decisions in patient care.Flexible Schedule: Enjoy a flexible on-call schedule that fits your lifestyle and provides the opportunity to make a difference on your terms.Supportive Team: Work with a dedicated team of healthcare professionals who are committed to quality care and patient well-being.If you are passionate about hospice care and making a difference in the lives of patients and families, we encourage you to apply for the On-Call After-Hours Hospice Nurse position today.Job Posted by ApplicantPro
