Job DescriptionThe Discharge Planner for MOUD (Medication for Opioid Use Disorder) and Medical Services is a key member of the correctional health team responsible for developing, coordinating, and implementing comprehensive discharge plans for incarcerated individuals. This role focuses specifically on ensuring a seamless transition of care for patients with medical needs, with an emphasis on those receiving or requiring MOUD. The Discharge Planner works collaboratively with a multidisciplinary team, including medical providers, mental health professionals, and correctional staff, to address the challenges of reentry and reduce the risk of relapse, overdose, and poor health outcomes post-release.Key Responsibilities:Assessment and Planning:Conduct thorough assessments of incarcerated individuals' medical and social needs to inform the development of individualized discharge plans.Identify patients who are receiving or would benefit from MOUD and prioritize their discharge planning needs.Collaborate with the medical team to establish discharge goals and objectives. * Coordination of Care:Maintain an up-to-date knowledge of community resourcesServe as the primary point of contact for external community providers and agencies. (MOUD and Medical)Facilitate appointments with medical providers, including primary care physicians, specialists, and MOUD prescribers, before release.Arrange for the continuation of MOUD upon release, including coordinating initial appointments and prescriptions.Secure appointments for follow-up medical care for serious and chronic conditions, including medication management.Connect patients with substance use treatment programs, peer support groups, and harm reduction services.Documentation and Communication:Document all discharge planning activities, assessments, and patient interactions accurately and promptly within the electronic health record (EHR).Create and provide patients with a "warm handoff" summary that includes a list of medications, scheduled appointments, and contact information for community resources.Communicate regularly with the correctional health team, correctional staff, and community partners to ensure a coordinated and effective discharge process. * Program Development and Evaluation:Participate in quality improvement initiatives to enhance the discharge planning process and improve patient outcomes.Contribute to the development of policies and procedures related to MOUD and medical discharge planning.Track and report on key metrics, such as the number of patients successfully linked to MOUD post-release and the rate of appointment attendance.Qualifications:Education: Degree in Nursing, or a related field, required.Experience: A minimum of 1 year of experience in case management, social work, or discharge planning, preferably in a correctional, hospital, or community health setting.Knowledge and Skills:Strong understanding of the principles of discharge planning and case management.Knowledge of substance use disorders, particularly opioid use disorder, and MOUD treatment modalities.Familiarity with community resources and social service agencies.Excellent communication, interpersonal, and organizational skills.Ability to work effectively with a diverse population, including individuals who have been incarcerated.Proficiency in electronic health records and Microsoft Office Suite.Ability to work independently and as part of a multidisciplinary team.If offered this position, your employment will be contingent upon passing a background check and drug testing. There will be no cost to you for these checks. This check will include fingerprinting, which will be available at various DHHS locations for your convenience and drug testing at a facility close to your location.   • Licensure as a registered nurse (RN) in the state of Utah.
  • Risks which require the use of special safety precautions and/or equipment, e.g., working around operating machines, working with contagious diseases or hazardous chemicals, etc.
  • Work requires physical exertion. May require the ability to stand; walk over rough surfaces; bend, crouch, stoop, stretch, reach, lift moderately heavy items (up to 50 lbs.) in a recurring manner and/or for long periods of time.
