Job SummaryThe MDS Coordinator is responsible for the overall coordination and management of the resident assessment process, including timely and accurate completion and submission of the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and Care Plans in accordance with HCFA and State of Connecticut requirements.
Key ResponsibilitiesAdministrative Functions
- Coordinate, complete, and transmit MDS assessments in accordance with state and federal regulations
- Maintain MDS/care planning policies and procedures
- Develop and monitor a quality assurance program for assessments and care plans
- Participate in discharge planning processes
- Chair the Interdisciplinary Care Plan Team
- Ensure accurate development of individualized care plans based on MDS assessments
- Conduct initial and periodic resident assessments
- Monitor significant changes and ensure appropriate follow-up MDS is completed
- Collaborate with all departments and participate in key facility committees
- Assist in maintaining communication and compliance between care team members
- Facilitate resident and family participation in care planning
- Provide education and training on MDS and care planning processes
- Assist with ongoing in-service programs and training needs
- Foster positive working relationships across departments and care teams
- Uphold resident confidentiality and rights, including advance directives
- Respond to and document resident complaints or concerns related to assessment or care plans
- Ensure assessments align with resident goals and preferences
- Nursing Degree from an accredited program required
- Active, unencumbered RN license in the State of Connecticut
- Minimum 2 years of supervisory nursing experience in a hospital or long-term care facility
- Prior experience in MDS coordination highly preferred
- Thorough knowledge of MDS 3.0, HCFA regulations, and CMS guidelines
- Excellent leadership, communication, and organizational skills
- Ability to work independently and collaboratively
- Strong assessment and documentation skills
- Familiarity with CEI software and electronic submission systems
