In this vital role, you will play a crucial part in coordinating and overseeing the care plans of our residents, ensuring they receive the highest quality of life possible.Responsibilities:
  • Conduct comprehensive assessments of residents' physical, emotional, and social needs, working closely with residents, families, and other healthcare professionals.
  • Develop and implement individualized care plans, utilizing interdisciplinary team collaboration to address all aspects of resident well-being.
  • Monitor and track resident progress, adjusting care plans as needed.
  • Advocate for residents' rights and ensure their autonomy and dignity are respected.
  • Provide education and support to residents and families on various aspects of aging and healthcare.

Qualifications:
  • Registered Nurse (LPN) OR (RN) WISCONSIN license with at least 1 years of experience in geriatric nursing, preferably in a long-term care setting.
  • Strong clinical skills and knowledge of geriatric healthcare needs.
  • Excellent communication, interpersonal, and collaboration skills.
  • Ability to prioritize and manage multiple tasks effectively in a fast-paced environment.
  • Strong organizational and time management skills.

  • Proficient in electronic medical records systems.
