We are looking for dedicated individuals to join our team and help deliver on our mission of hope and recovery.Position Purpose: Deliver mental health services & ancillary support services to clients who reside in Permanent Supportive Housing unit resources. Documentation and completion of reports to meet the requirements of Grant deliverables. Provide program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards.Job Requirements:
  • High School Diploma required, Bachelor's Degree preferred.
  • Minimum of one-year experience in mental health setting.
  • Valid Driver’s License with clear driving record and valid auto insurance.
  • Self-starter with ability to work independently

Essential Responsibilities:
  • Conduct individual skills teaching to assist individuals in obtaining &/or maintaining their home. Services could include (but not limited to) including the following:
  • Transporting clients
  • Assisting clients with medical appointments
  • Assist clients with moving & cleaning apartments
  • Coordinating apartment treatment procedures
  • Locating housing resources & communicating with landlords
  • Maintenance & upkeep of reporting system to track all deliverables provided.
  • Complete case notes with timeliness and accuracy.
  • Meeting clients within the community or at their homes to perform necessary skill trainings.
  • Serve as a member of the Homeless Services team and participate in all team meetings and activities.
  • Ability to motivate & problem solve with staff regarding effective clinical practices.
  • Ability to be self-motivated, seek improved performance, problem solve while meeting the needs to clients & their families.
  • Ensure that client health records are maintained consistent with rules & regulations.
  • Ability to coordinate services from a variety of community resources.
  • Skills to adapt verbal & written communication to the language & style of the client & their family.
  • Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
  • Deliver exceptional customer service consistently to every customer. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance
Schedule: 8-hour shifts
  • Monday – Friday 8:30am – 5:00pm
Work Location: Rosecrance Ware Center – Rockford, ILCompensation & Rewards Based on education, experience, and credentials
  • Starting pay - $18/hr
Shift differentials
  • Second shift: +$.25/hr.
  • Third shift: +$.50/hr.
  • Incentives:
